• Go to www.in.gov/idem

• Select "Permits", from the tabs at the top of the page.  Then select "Water" from the second
  row of tabs at the top of the page

• Select "IDEM Permit Guide" scroll down and select "Water Related Issues"

• Next find "NPDES Stormwater Discharges" and select "Construction Activities Rule 5 Permits"  - This will provide information and forms for the current Rule 5 and related links
